Measures total & free testosterone with SHBG and estrogen
Identifies imbalances driving fatigue, mood, or libido changes

This panel measures total testosterone, free testosterone, SHBG (sex hormone binding globulin), estradiol, and albumin.

Total testosterone measures all testosterone in the blood. Free testosterone — just 1–3% of total — is the biologically active fraction. SHBG binds testosterone and makes it unavailable. Testing all three together is the only way to know how much testosterone your body can actually use. Estradiol is included because it rises when testosterone is converted via aromatization.

This advanced testosterone test goes deeper than standard checks, measuring total testosterone, free testosterone, SHBG, estrogen, and albumin. Whether you're enhancing training, addressing low energy, or monitoring changes with age, this at-home testing kit will get you closer to your goals.

Is this test right for me?
  • You experience low energy, reduced libido, mood changes, or difficulty building muscle (Bhasin S et al., NEJM 2010 — these are established clinical symptoms of testosterone deficiency (hypogonadism).)
  • You are over 35 and want to understand how your testosterone levels compare to healthy ranges (Harman SM et al., J Clin Endocrinol Metab 2001 — total and free testosterone decline progressively after age 30 in men.)
  • You are currently on testosterone replacement therapy (TRT) and need to monitor your levels (Endocrine Society TRT Guidelines 2018 — testosterone, hematocrit, and estradiol monitoring every 3–6 months is recommended during TRT.)
  • You have been told your testosterone is “normal” but still feel symptoms — you may have low free testosterone despite normal total levels (Vermeulen A et al., J Clin Endocrinol Metab 1999 — SHBG levels significantly affect free testosterone; total T alone can be misleading.)
  • You are an athlete optimizing performance and recovery (Kraemer WJ & Ratamess NA, Med Sci Sports Exerc 2005 — testosterone is a primary anabolic driver for muscle adaptation and recovery.)
  • You want to understand how SHBG is affecting your bioavailable testosterone (Rosner W et al., J Clin Endocrinol Metab 2007 — SHBG-bound testosterone is hormonally inactive; free and albumin-bound fractions determine bioavailability.)

Measures Total & Free Testosterone with SHBG and Estrogen
This advanced testosterone test goes deeper than standard checks, measuring total testosterone, free testosterone, SHBG, estrogen, and albumin. Whether you're enhancing training, addressing low energy, or monitoring changes with age, this at-home test delivers physician-grade accuracy.
Research Evidence
Only 2–3% of total testosterone is free and biologically active — a total T test alone misses the fraction that actually drives energy, muscle, and mood
Free testosterone is the bioavailable unbound form and the key biomarker for optimizing health through testosterone therapy
SHBG regulates how much testosterone is available in the body — calculated free T using total T, SHBG, and albumin correlates at r=0.92 with gold-standard equilibrium dialysis
Research Basis
Testosterone circulates in three forms: free (2–3%, biologically active), albumin-bound (40–50%, weakly bound and bioavailable), and SHBG-bound (40–50%, tightly bound and inactive). Measuring all three is the complete clinical picture.

Identifies Imbalances Driving Fatigue, Mood, or Libido Changes
This advanced testosterone test goes deeper than standard checks, measuring total testosterone, free testosterone, SHBG, estrogen, and albumin. Whether you're enhancing training, addressing low energy, or monitoring changes with age, this at-home testing kit will get you closer to your goals.
Research Evidence
Estradiol (E2) is a form of estrogen that men need in small amounts — critical for bone health and libido, but high levels can lead to fat gain, mood changes, and water retention
Albumin is the primary protein produced by the liver — it carries hormones through the bloodstream, and low albumin may indicate liver dysfunction, inflammation, or inadequate nutrition
Elevated SHBG — common with aging — can make total testosterone appear normal while bioavailable testosterone is critically low, masking the real cause of fatigue or low libido
Research Basis
Someone can have normal total testosterone but low bioavailable testosterone if SHBG is elevated. This is why clinical labs recommend measuring free or bioavailable testosterone alongside estradiol whenever SHBG may be abnormal.

Monitor Changes with Age or Training
This advanced testosterone test goes deeper than standard checks, measuring total testosterone, free testosterone, SHBG, estrogen, and albumin. Whether you're enhancing training, addressing low energy, or monitoring changes with age, this at-home test delivers physician-grade accuracy.
Research Evidence
1 in 4 men experience declining testosterone levels — serial testing establishes when cumulative decline crosses the threshold where symptoms begin to emerge
Testosterone naturally declines 1–2% per year after age 30 — and as SHBG rises with age, bioavailable testosterone drops faster than total T, a shift invisible without both markers
For men on TRT, periodic retesting of total T, free T, SHBG, and estradiol is the clinical standard for dialing in dosing and monitoring for estrogen-related side effects
Research Basis
Declining bioavailable testosterone with age despite stable total testosterone is a validated clinical pattern caused by age-related SHBG increases. Sequential testing is the only way to detect this shift before symptoms become significant.

Frequently Asked Questions

What's included, how to collect, when you'll see results, and how to use them.

What does this panel measure?

Total and, where specified, free/bioavailable testosterone with SHBG (± albumin index) to evaluate androgen status accurately and trend levels over time for lifestyle or clinician‑guided programs.

How is this different from traditional testing?

Boomerang Kits deliver clinical‑grade accuracy with unmatched convenience: one at‑home collection, CLIA‑certified lab analysis, and secure app‑based results. No waiting rooms, no hidden telehealth upsells — just clear, trusted data.

Who benefits most from this panel?

Men monitoring energy, drive, and body composition — including those on physician‑guided testosterone replacement therapy (TRT).

Can I use this to monitor TRT?

Yes. Many users employ this panel for longitudinal tracking alongside clinician‑guided therapy, enabling data‑driven dose adjustments and safety monitoring.

Is this a diagnostic test?

No. This is a laboratory screening panel intended for education and wellness. Boomerang Laboratories does not diagnose, treat, or prescribe. Always consult a licensed healthcare provider for medical decisions.

How do I collect the sample?

At‑home finger‑prick dried blood spot (DBS) using the included lancets and collection card. Follow the insert or in‑app guide; collection takes ~10–15 minutes, plus ~60 minutes drying time before mailing.

How do I return my sample?

Use the prepaid return mailer. Drop at any postal location the same day you collect (after the card is fully dry).

What if my sample can't be processed?

If we can't process your sample (for example, under‑filled or insufficient), we'll notify you and send you a complimentary replacement kit.
